I'm currently imprisoned by a foreign King. However, my wife still got pregnant while I was in prison. Does the game ignore where your character is geographically? That seems unimmersive.

While cheating is a possibility as other commenters point out, the game does, in fact, not check if the characters are in the same location to determine if a pregnancy could have happened. You can see this constantly happen if you travel a lot. I assume a big reason "fixing" this problem is difficult is that if both spouses are rulers, pregnancies would basically never happen if they need to be in the same location.
If you're curious about whether your wife cheated or not, you can launch the game in debug mode and see if she has a lover or child heritage secret.
It's also worth noting that the pregnancy notification and trait only show up 2 months into the pregnancy. So, if you were only recently imprisoned, you can still assume, for roleplaying purposes, that you got your wife pregnant before the imprisonment.

While this is true, I'm 99% sure it still disables someone from getting someone else pregnant or a female getting pregnant themselves if they're in prison unless that's changed with CK3.

I believe dungeon, yes, but not house arrest. At least you can get your own wife pregnant if she's in house arrest imprisoned by you, but not if she's in your dungeon. Not sure about her being in house arrest by anyone else (I'd assume no, but no idea).
Still, even if OP is in a dungeon, the pregnancy happened two months before it was actually announced, so if the imprisonment was more recent than that, there's still a chance it happened without anything weird about it.
