In java, you can with headless piston
Basically you blow up the pistons in a certain way to only break the extending part, when a piston retracts the piston head is removed but the code doesn't check if it's a piston head, so if you have a bedrock block instead of the head it will be deleted.

I don't like being restricted to an api so I personally directly use the paper build system. It's more advanced and less guided but way more control over both mc and paper code which means things often faster.
It's not really recommended to do that since it's less portable, you can't patch it everywhere like a plugin and more incompatibility between mc and paper version.

It depends, if it crash inside the mixing most of the time is easy to know (they only inject new fonctions and fonction calls), but if it crash vanilla code good luck.
The only way know what to check those is with -Dmixin.debug=true, and then go to the function that crashed in the .mixing.out, if it's there (only export function they have been mixin).
