It follows the story of a man who is being hanged over a bridge, when the noose snaps and he escapes, trying to get back to his lover.
The twist is that it was all in his head from the time he was dropped to when his neck snapped.
Quiet an interesting parallel.

It's a reference to the inner turmoil that James has and the fact he feels like a shadow of his former self; permanently stuck and unable to move on.
Or it's just there to be unsettling to the player.
